• Junior High School (Grade 7 – Grade 10)

April-June 2016

Monday – Friday (8AM-4:30PM), Saturday (8AM-11AM)

• Senior High School (Grade 11)

 

April 18 – May 13, 2016

• School Enhancement Program for Incoming Grade 1

May 9 – May 20, 2016

• School Readiness Program for early enrollees in Pre-School

 

Requirements

Age requirements

• 3 years for Nursery
• 3.5 – 4 years for Pre-Kinder
• 4.5 – 5 years for Kinder

Pre-School

• NSO-authenticated Birth Certificate (SECPA)
• Report Card (photocopy)
• Recent 2×2 ID picture (2 copies)

Primary(Grades 1-6) and Junior High School (Grades 7-10)

• NSO-authenticated Birth Certificate (SECPA)
• Report Card
• Recommendation Form from Principal/Guidance Counselor of school last attended
• Recent 2×2 ID picture, two (2) copies

Senior High School (Grades 11-12)

• Junior High School (JHS) ID (for registration purposes)
• Philippine Statistics Authority (PSA)-certified Birth Certificate
• Certificate of Good Moral Character
• Placement Exam Result
• Recent 2×2 ID picture, two (2) copies
• Long size white folder

Additional Requirements for Public Junior High School Graduates:

• Grade 10 Report Card with Learner Reference Number (LRN)

Additional Requirements for Private Junior High School Graduates (ESC Grantees):

• Education Service Contracting (ESC) Program Certification letter from JHS Principal (format available through ESC IMS)

Additional Requirements for Private Junior High School Graduates (Non-ESC Grantees):

• Qualified Voucher Registration (QVR) Certification

Category C– (Transferees)

• NSO-authenticated Birth Certificate
• Transcript of Records
• Transfer Credentials
• Certificate of Good Moral Character
• Placement Exam Result from CSA-B or from school previously attended
• Recent 2×2 ID picture, two (2) copies
